An infrastructure for primary care to learn together with our patients

As CPIN learns from patients, the team will make data and findings available aiming to mobilize knowledge that informs and transforms practice, clinical care and decision making for better and more equitable health outcomes, especially for people with complex and/or unmet needs due to multiple, intersecting determinants affecting their health including linguistic minority status.

Critères de participation

Inclusion Criteria:
Patients: All adult patients of 18 years of age and older of participating primary care physicians. Patient must be able to read in French or English, or use a translation app, and have an email address or cellphone number documented in the practice EMR.

Primary care providers: All primary care providers who provide comprehensive care in Canada and whose practices participate in the CPCSSN initiative will be eligible to participate.
Exclusion Criteria:
Patients who do not have a cellphone number or email address will be excluded because this contact information is essential for the software to send the outreach communication messages. We have opted not to include minors, patients with advanced dementia, and people who cannot read and write in English or French and cannot use a translation App, because the automated approach to surveying patients makes it impractical to properly obtain their consent.
