The Mega Randomized Registry Trial Comparing Conservative vs. Liberal Oxygen Targets (Mega-ROX)
Mechanical Ventilation | Critical Illness | OxygenOxygen (O2) can be both lifesaving and toxic. All ICU patients receiving mechanical ventilation receive O2 therapy. Clinicians are more concerned about a life-threatening lack of O2, termed hypoxia. Accruing evidence now shows that excess O2 exposure, termed hyperoxia, can also cause harm. The adverse effects of hyperoxia may not be immediate. Alberta data show >80% of ICU patients are exposed to hyperoxia.
Accordingly, in a clinical trial we are comparing 2 strategies for O2 therapy in ICU patients receiving mechanical ventilation on mortality: usual (liberal) O2 targets vs conservative O2 targets. We are conducting this trial in multiple countries and an to enroll 40,000 critically ill patients.

Participation Criteria
Inclusion Criteria:
The trials aims to include any adult patient with an unplanned admission to an intensive care unit who is receiving invasive mechanical ventilation.
Exclusion Criteria:
There are no exclusion criteria other than clinician discretion on what is in the best interest's of the patient.
Patients will need to be enrolled within 12 hours of fulfilling the eligibility criteria. Where a patient is not enrolled within 12 hours of fulfilling the eligibility criteria, they will counted as “eligible but missed” rather than “excluded” for the purposes of describing participant flow.
