One:carepath Co-Design
Supportive CareAlthough, there are many common symptoms that occur with chronic conditions, there is a lot of variation in the supportive care these patients receive. Their care is fragmented and uncoordinated, and may not reflect their wishes. To improve care for patients with chronic conditions there needs to be a standardized approach to better coordinate services no matter where they live in the province.
Small focus groups and interview will be held to co-develop and integrated supportive care pathway for patients with chronic disease.

Critères de participation
Inclusion Criteria:
Input is required from those who provide or receive supportive care in a primary care setting. This includes primary care providers that care for patients with end-stage chronic illness and patients or family members of patients with a complex, chronic disease.
Exclusion Criteria:
Participants must speak English in order to participate, and be cognitively sound to provide informed consent and feedback.
