Trial of Intradialytic Cycling(aerobic exercise) as Kidney Exercise Rehabilitation for Cardiac Stunning in Hemodialysis
Chronic kidney disease (CKD) is common, In Canada, approximately 4 million individuals have CKD, and 43,000 have progressed to kidney failure.
Most of the 6000 Canadians who develop kidney failure annually start in-center hemodialysis.
HD treatments are associated with poor quality of life and adverse outcomes.
The HD procedure itself causes repetitive episodes of transient heart muscle injury (called stunning) and is associated with worsening functional status, increased heart failure rates, and sudden cardiac deaths.
The study will assess if participation in a 12-week aerobic exercise program during hemodialysis(HD) decreases HD-induced heart muscle injury (cardiac stunning).

Eligible Ages:
18 and up
Participation Criteria
Inclusion Criteria:
1. Adults > 18 years old (no upper age limit); who are > 3 months after starting maintenance HD
2.No expected change in dialysis modality, elective surgery, or relocation outside of Winnipeg during the intervention period (16 weeks)
3. Assessed to be safe and able to exercise by the HD unit nephrologist
4. Able to communicate in English and provide written informed consent
Exclusion Criteria:
1. Acute coronary syndrome in the past 3 months
2. Unstable arrhythmia
3. Shortness of breath at rest or with minimal activity (NYHA Class 4)
4. Symptomatic hypoglycemia (>2x/week in the week prior to enrolment)
5. Participating in an intradialytic exercise program within the last 3 months.
